Patient Rights

Patients have a Bill of Rights which is posted in our lobby. Patients who refuse to follow medical advise or exhibit unacceptable behavior may be dismissed from our practice. All patients are required to provide picture identification.

Payment Terms

We make every effort to keep the cost of your medical care down. You can help by following our office financial policies. Insurance co-payments and deductibles are to be paid at the time of service.

We offer a discount for private pay individuals if paid in full at time of service. If you are unable to pay at the time of service, depending upon your payment history, we may allow a short term monthly payment plan, however the discount policy does not apply.
